Getting Started

Once we've got you on board, here are some of the things we will need you to fill out to get us up to speed.

What We Need from You

Below are some starting paperwork documents and recurring documents we will need as we work for you as well as some documents required for us to set up Electronic Funds Transfers (EFTs) to get you paid quickly!

Initial Enrollment Info

This is a document asking for some general information about your practice and other information we may need.

Weekly Report

This is a document we will typically need from you every week confirming who you saw on which days and what payments you received so we can submit claims accurately and cross-reference payments to make sure you are getting paid what you should.
